Personal Information OPT OUT FORM

 Personal contact information published in the 2021-22 school directory will by default come from the school office. It will include child's name, parent(s) name(s), home address, home telephone number, and mother's & father's email addresses and cell phone numbers. Since most communication is done electronically, we encourage having at least one parent's email address published.

The deadline for information to be excluded the school directory is Friday, September 18th.
